Calls not going through for one area code only

I’ve got a bit of a puzzling problem where phone calls won’t go through normally when calling or receiving calls one specific area code.

Users have a 281 area code phone number from the Houston area.
They live in the 254 area code near Waco.
They use Starlink for their ISP and point of presence is in Dallas.
Any time they call a 254 area code number, they get the fast busy signal.
When someone from 254 area code calls them, they can hear the caller but the caller can’t hear them. No outbound audio.

It’s not an issue with the callers from 254 area code as there are many of them. Pretty unlikely all of the 254 callers have phone problems.
They can make/receive calls from all other area codes with no issues.
They can call from their 281 area code cell phones and the calls work just fine even when calling over a WiFi connection.

Any thoughts?
